GROUP INSURANCE ENROLLMENT FOR ORGANIZATIONS OF 25 OR MORE EMPLOYEES
It is becoming more common for companies to enroll groups of 25 employees or more without asking for individual enrollment forms. This is very helpful when you have employees spread all over the globe. But, although they do no ask for individual enrollment forms, companies will want the Insurance Administrator to fill in a form declaring any major medical conditions in the group, e.g. cancer, heart attack or chronic medical problem like diabetes.
