On plans below, the insurance company must be pre-notified within the first 90 days of pregnancy. Failure to pre-notify a pregnancy may cause a reduction in eligible benefits.
PLAN A (Citizen Series Secure)
Maternity – Normal or Complicated Delivery: After the Deductible, Underwriters will pay 50% of the next $100,000 of Eligible Medical Expenses, then 100% to a Lifetime Maximum of $250,000. Covered Maternity expenses include pre-natal, Delivery, and post-natal care (after 12 months of continuous coverage) Maximum for Maternity: $250,00 Lifetime Newborn Care: Included as part of Maternity benefits for a maximum of 60 days

Benefits
include: • Pre- and post-natal care • Maximum of US$5,000 for normal delivery
for each pregnancy • Maximum of US$7,500 for C-section delivery for each
pregnancy • Well baby care and treatment of newborn for first 31 days • Child
wellness benefits of up to US$200 maximum per period of coverage (not subject to
deductible or coinsurance) for eligible newborn children for the first 12 months
Benefits available after 10 months of continuous coverage • Eligible newborn children may be added without evidence of insurability as long as an application form is submitted within 31 days of birth • Benefits will be reduced by 50%

PLAN D (Reside Worldwide Health Plan-non health insurance plan)
Maternity
Normal and complicated child delivery maximum, including pre- and postnatal
care is reimbursed according to the other medical treatment benefit
schedule. Waiting period of 12-months before maternity benefit begins.
Professional service related to hospitalization, per day with in the US and
Canada is $200 and outside the US and Canada is $250. Treatment received
inside the United States and Canada is $2,500 per pregnancy and treatment
received outside the United States and Canada are $4,000 per pregnancy.
Newborn coverage
Lifetime maximum for the first 31 days after birth, per limits as stated in
the Certificate of Coverage. US $5,000 treatment received inside the United
States and Canada and US$10,000 for treatment received outside the United
States and Canada.

Maternity Benefits:After 12 months of continuous coverage, Global Citizen members may renew
their coverage or apply for a new plan that covers maternity costs in the
same way as all other medical conditions.
To be eligible for the maternity benefit, a member must not be pregnant at
the time of upgrade.
Average monthly maternity rider, depending on age, is from $70 to $100.

PLAN G (Citizen Secure Economy)
Maternity: $5000 per Pregnancy after 12 months of continuous
coverage hereunder, including
Inpatient, Outpatient and other benefits as herein provided. Not subject to
Coinsurance.
Newborn Care: $15,000 per covered Pregnancy, including Inpatient, Outpatient and
other benefits as herein provided, during the first 60 days of life.
