The following international health coverage plans provide: $5,000,000 maximum lifetime coverage; 80/20 co-insurance within the USA, no co-insurance outside the USA; PPO networks inside the USA and acceptance of any doctor outside the USA. Deductibles from $250 to $10,000. International medical evacuation coverage included. In these plans the usual, reasonable and customary reimbursement rate is used for hospitalization, out-patient surgery and all medical care. Prescription drugs are covered after the deductible is met.

Highest deductible on this overseas health insurance plan is $2,500. If no riders/waivers, all pre-existing conditions covered. Mental health disorders capped at $25,000 lifetime. Limited wellness coverage. Some human/organ transplants covered, and are treated the same as any other illness. Benefits cut 50% if company not pre-notified prior to receiving medical care. Limited life insurance option available highest deductible on this overseas health insurance plan is $2,500. If no riders/waivers, all pre-existing conditions covered. Up to $1,000,000 transplant coverage in designated transplant facilities. Medical evacuation to policy limit. The first $300 covered for each accidental injury. Limited coverage of $5,000 per policy period for pre-existing conditions after 24 months on plan. Maximum of $25,000 for mental-nervous care. If 35 or over, wellness after 12 months on policy, $250 per policy period. Limited coverage for complementary medicine, e.g., acupuncture, herbal therapy, etc. Chiropractic, radiation treatment, home nursing care, hospice care, physical therapy also covered. More cost effective than Plans A-C and very similar to limits on coverage as in Plan D with these added benefits: Some well child care, medical evacuation of $50,000, extended care facility services, home nursing care, inpatient hospice care and return of mortal remains. Maternity rider costs an extra $2500 per year. Key disadvantage is that newborn care is limited to $15,000 for the first 30 days after birth. The plan is a bit less expensive than Plan D.
