You are free to use any doctor in the world as long as they are a licensed professional.
No, HCC does not require pre-notification for simple doctor visits. Contact HCC in advance or within 48 hours for the following:
- Inpatient Care
- Surgery or Surgical Procedure
- Extended Care Facility
- Home Nursing Care
- Durable Medical Equipment
- CAT scans & MRIs
- Repatriation of Remains
- Emergency Medical Evacuation
Emergency Evacuations must be pre-notified in advance and coordinated by HCC to be considered for coverage.
Eligible Medical expenses will be reduced by 50%, and the deductible will be subtracted from the remaining eligible amount, and the Benefit Percentage will be applied. If Pre-notification requirements are not met for transplant treatment, benefits are forfeited for all services or supplies for the Transplant.

No. Although you’ve completed the Pre-notification Requirement, all plan provisions and conditions must be met at the time of treatment. We cannot authorize, or guarantee any benefit prior to the service. Pre-notification is not a guarantee of benefits.
Yes, in most cases you will need to pay the doctor up front and then submit a claim form to HCC for reimbursement.
You may obtain a copy of the claim form by visiting www.mnui.com . If you do not have access to the internet, please feel free to contact our customer service department so that we may send a form to you by fax or by mail.
No, you should submit one Claim Form to HCC for each different condition or diagnosis only.
Drugs will only be paid for if they are not related to a pre-existing condition and if you have reached your deductible. Prescriptions should be paid for at the pharmacy then submitted to the insurance company as a claim for reimbursement. Please include the prescription label and your paid receipts with a completed claim form.
Yes. We can define the benefits that are available within your coverage; however, we cannot pre-approve any treatment or guarantee payment in advance.
The claims department needs at least 10 business days to set up a file and begin reviewing a submitted claim. If additional information is needed to process the claim, written requests will be sent at that time. After all required information is received and reviewed, the claims will be processed according to the terms of the insurance.
You are welcome to contact us during regular business hours (7am to 6pm Eastern April through October; 8am to 7pm Eastern November through March) to inquire about the status of a claim.
You may ask for HCC to reconsider the denial by submitting a request for an appeal in writing within 90 days. In order for the claims department to review the appeal, you must supply additional documentation to support a reversal of the denial.

HCC coordinates all evacuations through its home office in Indianapolis, IN and arranges for medically necessary evacuations through various service providers in all regions of the world.
