How Coinsurance Affects Your Out-of-Pocket Maximum

What coinsurance covers after your deductible, and how the cap works.

A family relaxing together at home, reading a book on the couch.

Since 1997, GNI’s team has fielded thousands of questions about how expat medical insurance actually pays out. One that comes up often: once you’ve met your deductible, what happens next — and how does coinsurance connect to your out-of-pocket maximum?

What Is Coinsurance, and How Does It Fit In?

Coinsurance is generally the percentage of eligible covered expenses you may pay after the deductible has been met. Depending on the plan, other cost-sharing amounts may also apply.

For a fuller walkthrough of how coinsurance fits alongside deductibles and copays, see our Copay, Deductible, Coinsurance: Expat Terms Explained guide.

How Does Coinsurance Work With My Out-of-Pocket Maximum?

Some expat medical insurance plans include an out-of-pocket maximum that limits certain eligible cost-sharing amounts, such as coinsurance, after the deductible has been met. The plan documents should explain which expenses count toward that maximum. Once the applicable out-of-pocket maximum has been reached, the plan may pay eligible covered expenses at 100%, subject to the policy’s terms, exclusions, benefit limits, and any expenses that do not count toward the maximum. The exact percentage and cap depend on the plan and carrier.

Depending on the plan, different coinsurance or cost-sharing rules may apply when care is received outside the plan’s preferred provider network. Review the network provisions before receiving care when possible.

Some plans may apply different coinsurance or cost-sharing rules based on where eligible care is received. For example, an expat medical plan may have different terms for treatment outside the U.S. than for treatment inside the U.S. Confirm the details in your plan documents or with the carrier before relying on a specific outcome.

Why Does My Family’s Out-of-Pocket Maximum Look Higher Than One Person’s?

Some international medical insurance plans apply an individual out-of-pocket maximum to each covered person and may also set a separate family maximum. The family maximum may be based on a multiple of the individual amount, but the exact structure, amount, and calculation method depend on the carrier and plan. Confirm these details in the certificate of coverage or policy documents.

Once you understand your plan’s individual and family out-of-pocket maximums, you may have a clearer estimate of applicable cost-sharing for the policy or calendar year. However, expenses that are excluded, limited, non-covered, or outside the maximum may still remain your responsibility.

A Few Quick Questions

What does coinsurance mean on my expat medical insurance?

Coinsurance is generally the percentage of eligible covered expenses you may pay after the deductible has been met. Depending on the plan, other cost-sharing amounts may also apply. Browse GNI’s expat medical insurance plans to compare how different plans structure coinsurance.

How does coinsurance work with my out-of-pocket maximum?

Coinsurance and certain other eligible cost-sharing amounts may count toward your out-of-pocket maximum, depending on the plan. The policy documents should identify which expenses are included and which are excluded.

Once the applicable maximum is reached, the plan may pay eligible covered expenses in full, subject to the policy’s terms, exclusions, benefit limits, and expenses that do not count toward the maximum. Connect with a GNI advisor to see how this could apply to your situation.

Why is my family’s out-of-pocket maximum higher than an individual’s?

Some family policies set a family out-of-pocket maximum based on a multiple of the individual amount, but the calculation varies by carrier and expat plan. Review the policy documents before relying on a specific amount. Request a quote to see how this could work for your family.

Every expat medical insurance plan structures coinsurance and out-of-pocket maximums a little differently. Browse GNI’s expat medical insurance plans, or connect with a GNI advisor with any questions along the way.

Benefits, exclusions, eligibility rules, coverage limits, lookback periods, coverage area, restricted or excluded countries, and pre-existing condition provisions can vary by carrier and plan—and may change over time. As we like to say, change is always in the air, so use the guidance above as a helpful starting point, not a guarantee of coverage. Good Neighbor Insurance advisors can help explain available options and application requirements; however, the insurance carrier makes the final decisions regarding eligibility, underwriting, policy issuance, exclusions, benefits, premium rates and payment requirements, and claims under the policy.

Insurance plan details and insurance company processes may change from time to time. Please connect with one of our GNI international insurance advisors for the most current information.
