Claims Process
A claim must be submitted to IMG for each new onset of an illness or injury. Although IMG often works with hospital and clinic providers (including those outside their PPO) to pay directly to the provider, a claim is always required. This tells IMG you are receiving medical treatment and allows them to watch for appropriate billing to be filed on your behalf. If you fail to submit a Claim Form and additional information is requested, IMG will contact you with an Explanation of Benefits and attach a Claim Form if one is required.
A claim should be submitted as soon as you receive the necessary medical bills and/or receipts. If you have paid for services, you have 90 days to submit a claim with receipts to IMG from the date when service was provided. Do not wait until the end of the year to submit claims. IMG will apply eligible medical expenses to your deductible and coinsurance throughout the year.
Claims generally take four to six weeks to complete, if IMG has all the paperwork to process the claim. It may take longer if IMG must have medical documents translated into English or IMG requests medical records from the medical provider, facility or insured before the claim can be completed.
Claim Appeals
A written formal claims appeal may be sent to IMG requesting a review of previously processed claims. It is important that you submit your appeal and any supporting documentation within sixty (60) days of the original claim determination. Your appeal will be reviewed and responded to according to your insurance plan.

Claim Submission Options
IMG offers a few options for submitting a claim:
Online: IMG’s website imglobal.com offers the ability to submit claims and view your claims by logging onto MyIMG℠. First-time users can click here to go to the New User Registration website. Once registered, you will have access to your documents, claims status, frequently asked questions, printable ID cards, pre-certification and renewals. A separate ID and password will be set up for each member of your family.
Hardcopy: Print and complete the Claim Form and submit it with the original receipts. You can send them to IMG one of three ways:
Email: Scan copies of claim form and receipts and email them to [email protected] (make sure the subject line has the word “Claims” on it).
Fax: If you have a fax machine, you may fax your claim and receipts (make sure you indicate “Claims” somewhere on the cover page). The fax phone number is 1-317-655-4505.
Postal Mail: Mail your original claim and receipts but keep copies of them for your records. Mail them to:

Claim Form
The claim form requests the following information:
- Identification info about claimant
- Description of the new condition, injury or illness
- Treatment received outside the U.S.
- Payment info
- Your signature and the date
Be sure to gather all invoices/receipts from medical providers, medical facilities and for prescriptions.
If you have paid the expenses and IMG is reimbursing you, instead of the physician or hospital, indicate the address where the check should be sent to. This is very important and will cut down on the mail delay in getting your money to you.
