BCBS Global Solutions Single Trip Platinum Health Insurance for International Travel up to Age 95

• Must have U.S. domestic insurance or Medicare Part A to be covered on this travel insurance plan.
  (Medicaid, VA plans and “medical share plans” do not qualify as U.S. domestic insurance)
• Covers pre-existing health conditions.
• Any coverage for maintenance medical care and maintenance Rx is not covered.
• Maintenance medical care also includes medications prescribed for chronic and/or long-term medical conditions or Rx taken on an ongoing basis.
• Any drugs, medications, or other substances except as specifically stated in this Plan are excluded. This includes, but is not limited to, items dispensed by a Physician.
• Rx Examples: birth control, heartburn/acid reflux medication or anti-depressants.
• Medical maintenance examples: Needing blood transfusions on a continual basis for diabetes.
• Any ongoing medical condition or past medical condition that needs to be monitored and/or checked on a regular basis.
• Treatment of mental disorders or functional nervous disorders are not covered under the policy
• Policy holder cannot be disabled at the time of travel and cannot be on a waiting list for any treatment
• Policy holder cannot travel against the advice of a physician / medical facility.
• Single Trip Platinum is available in 47 U.S. states and DC.
• Complications of pregnancy are covered

Deductible per Insured Person per policy period:
Options include $0; $100; $250; $500

Eligibility Requirements:

• Home Country is the U.S.; and
• You must be under Age 95 and;
• Enrolled in a Primary Plan*; and
• For Children under age 6, must be enrolled with a parent and;
• Initial purchase must be made in home country prior to departing on trip

Family Members included in your Plan (if applicable) must be:

• Your spouse; civil union partner or domestic partner age 95 or younger, and/or
• Your eligible child(ren) or other eligible dependent(s)—see Eligible Dependents.
• Resident(s) of the United States.
• Traveling outside the U.S. and scheduled to spend at least 24 hours away from his/her home.
